SAFETY COME SECOND NOT FIRST

 Today,one bangladish worker died and one indonesian worker hurt when a concrete precast  pile hit them both.Actually normal at the site during the piling driven process only 2 or 3 piling workers will handle and supervise the process.The piling machine wiil pull the pile aat the hook and lift it to the position.Below are step by step process of driven

1. The machine will move and move near to the piling point
2. The machine will pull the pile with the cable at the hook point
3. The pile will move and lift to the position of pile
4. The machine start driven feet by feet until set
5. Workers scope of works are    

•   Mark the red line equal to 1 foot along the body of the pile
• Check the verticality of pile with waterlevel put on the surface of the concrete
• Pull and set the pile shoe to the position
• Make a welding to the connection
• Put a graph paper for set reading

So how the pile can hit the worker,maybe must investigate first

• The cable may be broken or fail
• The steel hook may be broken or in rusty 
• condition
• The cable hook not grib well the pile hook
• The machine driver or operate by the bangladish or not competent person
• The lack of safety precaution supervision
• Playing at the site while machine still in work progress

Actually the process to pick up the pile is

• The cable from the machine will give to the workers to hook to the pile and then lift it to the pile head and lock it and start driving.
• Maybe the pile stockpile far away from the point so that the cable stretch longer and longer and the broke when lifting and hit the nearby worker.
• This is the first time worker died hit by a concrete pile

Safety precaution

• Check all the cable and hook in good condition
• Change its if necessary
• PPE to all workers must be compulsory
• Don t let the uncompetent person drive the machine
• A full time supervisor locate at site for every all of time during driving process
• Don t let the workers do the work without SOP or guidance

Investigation are carry out to look what is the main cause of this accident to avoid happen again in the future.
